Collaboratieve somatische zorg voor personen met een ernstig psychische aandoening binnen 2b-teams
Opzet
Somatische co-morbiditeit komt frequent voor bij personen met een langdurig ernstige psychische aandoening (EPA), met een verminderde levensverwachting van 15 tot 30 jaar (Delespaul, 2013). Sinds de hervormingen binnen de GGZ worden personen met EPA begeleid door mobiele outreachteams (2b-teams). Hun werking is veelal gebaseerd op het Nederlandse FACT-model (flexible assertive community treatment). FACT-teams bieden zowel begeleiding als behandeling aan cliënten, waaronder ook somatische screening en farmacologische zorg. De 2b-teams in Vlaanderen beperken zich eerder tot psychosociale interventies en schenken nog onvoldoende aandacht aan somatische zorg voor deze doelgroep. Met behulp van het collaborative care model kan het mogelijk zijn op gepaste wijze personen met een EPA somatisch op te volgen (Martens et al, 2018).
Doel van het onderzoek is dan ook na te gaan of collaboratieve somatische zorg vanuit de 2b-teams effectief is op verschillende geïntegreerde cliëntgerelateerde domeinen. Bijkomend doel is het trainen van 2b-teams in de werking van het ontwikkelde model.
